In this episode, we unpack one of the most misunderstood areas in respiratory physiology:

Ventilation. Perfusion. Respiration.

And how they integrate through ventilation–perfusion (V/Q) matching.

We break down:

Ventilation — the mechanical movement of air driven by pressure–volume relationships
Perfusion — blood flow through the pulmonary capillaries
Respiration — the transfer of oxygen and carbon dioxide across the alveolar membrane

From there, we explore V/Q coupling and why the resting ratio sits around ~0.8 — the dynamic balance between airflow and blood flow inside the lungs.

Then we move deeper.

Into alveolar stability.
Into surfactant.
Into why small lung units collapse throughout the day due to posture, compression, or shallow breathing.

And why reopening them requires increased inflation pressure.

This is where sighing becomes physiologically important.

A sigh isn’t a trendy technique.
It’s a breath two to three times larger than normal tidal volume.

That larger inflation:
• Increases pressure
• Redistributes ventilation into smaller airways
• Re-expands collapsed alveoli
• Allows surfactant to stabilise them again

This is how mechanics, gas exchange, and functional breathing behaviour connect.
